The NAMI Illinois State Office hopes that the links above will assist you in several ways.   Our goal is to make your job as affiliate president easier.

We have provided:

• Many of the forms you use for submitting membership information
• Quarterly Stat form- submit in Oct., Jan., April, July of each year
• Application for Internal Revenue Service 501 (c)(3)
• Illinois Sales Tax Exemption
• Articles of Incorporation
• Publications available
• And important dates

When using the forms, you will need to print them off, then complete the requested information and submit them to the state office.  We hope in the near future, to be able to have the forms available to be submitted through the internet.
